Must be completed before a student can checkout a device from Madison High School. 

Madison High regularly highlights students in action in our classrooms, on the athletic fields or other extra-currirular activities.  In order for your student to be highlighted this form must be completed. 

This form must be completed EACH school year to be in compliance for us to refuse the military access to your student's records. 

Enrollment is voluntary. 

Your Advantage Card must be enrolled every year.
